Arjuna thought himself to be the doer of action, but he did not consider the supreme sanction within and without.
BG 18.17, Purport:

In this verse the Lord informs Arjuna that the desire not to fight arises from false ego. Arjuna thought himself to be the doer of action, but he did not consider the supreme sanction within and without. If one does not know that a supersanction is there, why should he act? But one who knows the instruments of work, himself as the worker, and the Supreme Lord as the supreme sanctioner is perfect in doing everything. Such a person is never in illusion. Personal activity and responsibility arise from false ego and godlessness, or a lack of Kṛṣṇa consciousness. Anyone who is acting in Kṛṣṇa consciousness under the direction of the Supersoul or the Supreme Personality of Godhead, even though killing, does not kill. Nor is he ever affected by the reaction of such killing. When a soldier kills under the command of a superior officer, he is not subject to be judged. But if a soldier kills on his own personal account, then he is certainly judged by a court of law.

The Bhagavad-gītā confirms that the Lord is situated in everyone's heart as the witness, and as such He is the supreme director of sanction.
SB 2.9.25, Purport:

The Bhagavad-gītā confirms that the Lord is situated in everyone's heart as the witness, and as such He is the supreme director of sanction. The director is not the enjoyer of the fruits of action, for without the Lord's sanction no one can enjoy. For example, in a prohibited area a habituated drunkard puts forward his application to the director of drinking, and the director, considering his case, sanctions only a certain amount of liquor for drinking. Similarly, the whole material world is full of many drunkards, in the sense that each and every one of the living entities has something in his mind to enjoy, and everyone desires the fulfillment of his desires very strongly. The almighty Lord, being very kind to the living entity, as the father is kind to the son, fulfills the living entity's desire for his childish satisfaction. With such desires in mind, the living entity does not actually enjoy, but he serves the bodily whims unnecessarily, without profit. The drunkard does not derive any profit out of drinking, but because he has become a servant of the drinking habit and does not wish to get out of it, the merciful Lord gives him all facilities to fulfill such desires.

Because without the sanction of the Supreme, I cannot do anything, so if we persist to do something, then the sanction is given, "At your risk." That is going on.
Lecture on BG 4.17 -- Bombay, April 6, 1974:

You cannot hide yourself from, any vikarma or unlawful work, from the eyes of the Supreme Lord. That is not possible. You can hide yourself from the eyes of the police, man-made law, but it is not possible to hide yourself from the eyes of the Supreme. That is not possible. Because the Supreme is sitting within your heart. Īśvaraḥ sarva-bhūtānāṁ hṛd-deśe 'rjuna tiṣṭhati (BG 18.61). And He is sitting as anumantā and upadraṣṭā. He is simply seeing what you are doing, and He is giving sanction also. Even a thief who is going to act something criminally, without the sanction of the Supersoul, who is sitting within everyone's heart, he cannot do that.

That you have got experience. Suppose you are doing, going to do something which is not very good. The conscience is beating, "No, no, you should not do this. You should not do this." But because without the sanction of the Supreme, I cannot do anything, so if we persist to do something, then the sanction is given, "At your risk." That is going on. God does not give you sanction for doing anything criminal. But if we persist to do something criminal, then God gives sanction, "All right, do. Do it at your risk." That is going on. Karmaṇā daiva-netreṇa (SB 3.31.1). Now, when we act something on account of my persistence by the sanction of the Lord, then I become subjected to the fruits of such resultant action.

So there is soul and there is the gross material body and there is the subtle material body. The soul is the basic principle, but to get a body, as I have already explained, the secretion discharged by the father and the mother, they mix up, they are emulsified and forms in the body of a pea. And the soul comes through the semina of the father and he's situated there. Then the body develops. Now, just try to understand. Because the spirit soul is there, therefore matter is developing. If the soul is not there, if the child is dead, no more development. No more development. No dead child develops a body. Everyone knows. Therefore these material elements come from the spirit soul, not that spirit soul comes from the material elements. This is not. This is wrong theory. If it comes from the material combination, then why you cannot produce a living entity in the laboratory? In the laboratory, no, that is not... A material... Because... Lecture on SB 1.2.8 -- Vrndavana, October 19, 1972:

Bhāgavata says that you cannot rectify the destiny of another man. That is not possible. Bālasya neha śaraṇaṁ pitarau nṛsiṁha. It is not that because one has got good parents, therefore he will be happy. No. Not necessarily. Bālasya neha śaraṇaṁ pitarau nṛsiṁha. So similarly, it is not that a diseased person, because he is being treated by a first-class physician and he is being supplied first-class medicine, therefore he will be cured. No, there is no such guarantee. Because if the supreme authority does not sanction... Suppose a man is diseased; he is going to die or suffering. So his relatives and friends are trying to save him. The śāstra says that "You cannot save him simply by giving him first-class medicine or first-class medical treatment." They, they can also, cannot guarantee. Ask any qualified doctor, that "This man is being treated by you. Can you guarantee that he will be cured?" They will say, "No, that is not possible. We are trying our best." Therefore we should know the ultimate sanction depends on Kṛṣṇa.

Sri Caitanya-caritamrta Lectures

In the midst of worshiping other demigods, Viṣṇu is presented, and that is the system of Vedic process. So therefore Viṣṇu is Supreme. Why Viṣṇu's sanction is required?
Lecture on CC Madhya-lila 20.144-146 -- New York, December 1, 1966:

Even there is worship of any other demigod, just like Durgā-pūjā, so even in that paraphernalia of gorgeous worshiping arrangement, when the worship will begin, the first worship is offered to Viṣṇu, Yajñeśvara, because, as it is stated in the Bhagavad-gītā, kāmais tais tair hṛta-jñānāḥ yajante anya-devatāḥ: (BG 7.20) "Those who are mad after material sense gratification, they go and worship other demigods to have some immediate reward." And they get it. But how they get it? That is also stated in the Bhagavad-gītā, mayaiva vihitān hi tān: "They cannot have that reward without My sanction." Because the demigods, they are also subordinate servants of the Supreme Personality of Godhead; therefore the demigods, although they can give you the reward which you want, but with the permission of the Supreme Lord... Because they are not fools. If somebody, some of you, somebody comes, outsider, and asks from this institution, our Society, that "Please give me this," so you can deliver it, but you will take permission from me. That is a common custom. Similarly, the demigods also, they cannot offer their reward without sanction of Viṣṇu. Mayaiva vihitān hi tān. Antavat tu phalaṁ teṣām (BG 7.23). Therefore Viṣṇu is present there. He... In the midst of worshiping other demigods, Viṣṇu is presented, and that is the system of Vedic process. So therefore Viṣṇu is Supreme. Why Viṣṇu's sanction is required? Therefore it is understood, although in different Purāṇas different types of worship for different types of demigods are recommended, but the ultimate sanction is of Viṣṇu. Therefore Viṣṇu becomes supreme.

Anyway, Dr. Urquhart's argument was that "Who is the witness? I am suffering the reaction of my previous bad or evil activities, but who is the witness?" But at that time we were not so intelligent. We could not answer. But later on, when we were grown up and studied Bhagavad-gītā, then here, in the Bhagavad-gītā, we saw that upadraṣṭā. The Lord is upadraṣṭā, He is witness. Upadraṣṭā. Anumantā. Anumantā means ordering. You cannot do anything without being sanctioned by the Supreme Personality of Godhead. You have no power. Therefore we are, in all respect, we are dependent. That we have got very nice experience. This hand is moving, but if the power is withdrawn, I cannot move my hand. Therefore I am not independent to move my hand. So upadraṣṭā anumantā. We cannot do anything without being sanctioned by the Supreme Lord. There is an English word, that not even a grass moves without the sanction of the Lord. So that is a fact. So how one is doing nice thing and how one is doing evil things if He is the order giver? That is our independence. We can take sanction from the Lord. If we want to do something evil, I cannot do it without the sanction of the Supreme. Or even if I do something very nice, that also I cannot do without the sanction.

Now, the question may be that whether one can finish all sorts of sinful reactions within this material world. No, that is not possible. But it is possible also. What is that? That is also answered in the Bhagavad-gītā that "You give up everything. Surrender unto Me. I shall give you protection from all sinful reactions." So I may be sinful and there are so many reactions awaiting me for giving me trouble, but as soon as Kṛṣṇa takes charge of me, then everything is finished. How it can be finished? Because He is all-powerful, He's the supreme. Just like if the president says... One man is ordered to be hanged.
